New Patient Registration

If you are new to the area and would like to register as a patient with us, all you have to do is pick up a registration pack from Reception, complete the registration and consent forms and hand them in to us with photo ID and proof of address.

• ​You will need to provide your NHS number on the form, otherwise we cannot register you. If you don't know your NHS number, please ask your current surgery or find it at the top of your prescription. You will need a separate registration pack for each member of the family.

• If you are over 16 years of age when you register, please book a 20 minute appointment for a New Patient Health Check when you hand in your registration form. These are free health checks for all new patients. Our healthcare assistant will discuss your general health, and check your blood pressure, weight and check a urine sample.

• Your medical records will be requested from your current GP surgery.

• If you are a new patient on warfarin please ask your previous practice to transfer your records over on INR Star BEFORE you register here. For more information on our anti-coagulation clinic please click here. 

 

Our practice boundary area for new patients covers Caddington, Slip End, Woodside, Aley Green and Pepperstock. We are only able to accept new registrations within our practice boundary area. If you move out of this area, you will need to register with a local doctor in your new location.
