If you take medication regularly, you will receive a tear-off slip with each prescription, listing the items you have been prescribed for repeat use and the date at which you next need to see  the doctor for review .Please use this slip to request repeat supplies: tick the items you need and return the slip to the surgery.

Most of our patients choose to use the Medigreen pharmacy which is next door to the surgery, and so we send on signed prescriptions there for dispensing. If you do not wish to use this chemist, please add a note"keep in surgery" to your prescription request so that you can collect the signed prescription form from the surgery and take it to the chemist of your choice.

Repeat prescription requests will not normally be taken by telephone as this can lead to mistakes.  Please allow at least 2 working days before collecting your prescription.
