New Patient Registration

If you have moved into Caddington, Slip End, Woodside or Pepperstock and would like to register with us, please come along to the surgery and fill an a Registration Form for each member of the family.

You will be asked to make an appointment for a New Patient Health Check. These are free health checks for all new patients. The practice nurse will discuss your general health, and check your blood pressure, weight and check a urine sample.

Your medical records will be requested and sent to the surgery in due course.

There is parking space in front of the surgery, and access and toilet facilities are suitable for the disabled. 

There is a detailed map of our practice boundary area at the surgery and we are only able to accept new registrations within our practice boundary area. This map shows the approximate area covered by the practice.
